Hospital to home - supporting your journey

After an illness or injury that requires a hospital stay, you may find yourself in need of some practical support to help you transition back to health and regain your independence at home. Often, hospital discharge will only be able to take place if the person returning home can do so safely with the right care in place.

As a local provider operating around Richmond, Twickenham and East Sheen, we’re flexible enough to offer whatever care is needed, specific to the individual, even at short notice. This might look like an hour a day or 24 hours of live-in care.

Our local care assistants, that we match to the personalities of our clients, have the patience and skills required to encourage a return to independence. They sensitively recognise the emotional impact that the illness or injury would have had on you, your family and friends and support this with coping strategies.

How does hospital discharge care work?

Typically, our clients for hospital discharge care tend to be either due in hospital, currently an in-patient, or already at home requiring support. They generally require, and we’re able to help with:

  • Escorting to doctors’ appointments
  • Collecting repeat prescriptions
  • Helping with personal care (e.g. bathing)
  • Assisting with household chores (e.g. cleaning and meal preparation)

 

Helping with hospital to home rehabilitation is dependent on the stage of recovery and in most cases tends to reduce as people start to recuperate. We work with you and your families on a hospital discharge care plan that works best for all involved.
